New Name

Please enter your NEW name, including any first name, middle name, and surname. Please ensure correct use of capital and lowercase letters, as well as correct use of any punctuation (eg." - "). It is your responsibility to submit your new name and title correctly.

Please give us your home address. This is usually the same as your billing address.

Note: We can only deliver to UK addresses right now. If you live outside the UK, we're sorry but we can't ship to you yet. We hope to offer international shipping soon!

Additional Legal Copies

Please enter the number of additional legal copies you would like to receive. We recommend a minimum of 10 additional copies. Typically, photocopies are not accepted.

Please note: You will have to send your Deed Poll to all record holders in order for them to amend their records. Most record-holders require that you submit a certified copy of your original Deed Poll.
